FAREWELL SUPPORT POLICY TERMS & CONDITIONS

Eligibility:

The Primary Assured must be a healthy client of Naa Sika, aged 18 to 60. The spouse and up to four children of the Primary Assured are automatically covered, with certain age restrictions of up to 21 years if still in school. Parents and parents-in-law of the Primary Assured, below 75 years of age, can also be insured. Assureds other than the Primary Assured are called 'Secondary Assureds'.

Incontestability:

SIC Life will not contest the validity of this policy after two years from the commencement date, except for non-payment of premiums.

The effective date of cover:

The insurance coverage starts on the first day of the month on which the premium is paid.

Termination of the insurance:

The policy will be terminated upon payment of the benefit in the event of the death of the Primary Assured. Non-payment of premiums beyond the reinstatement period will also result in termination.

Benefit:

Benefits are payable in the event of the Primary Assured or Secondary Assured(s) death due to accident or natural causes during the policy period. The benefit due to the Primary Assured is paid to the nominated beneficiary, while the benefit due to a Secondary Assured is paid to the Primary Assured. "Waiting periods" must be satisfied before the payment of benefits. Specific benefit amounts are provided based on the chosen option.

Beneficiary:

The Primary Assured nominates a beneficiary at the start of the policy. The nominated beneficiary can be changed through written notice to the company. If the nominated beneficiary predeceases the Primary Assured and no replacement is named, the benefit will be paid to the Primary Assured's estate.

Premium:

Premiums are payable monthly in advance. Non-payment of premiums after the grace period of 30 days constitutes a default under the policy.

Grace Period:

A grace period of 30 days is allowed to pay each premium. Benefits remain in force during this period, but unpaid premiums will be deducted from any payable benefits.

Reinstatement:

The policy can be reinstated within four months from default by remitting all overdue premiums. Reinstatement is binding upon the company, and a four-month revived waiting period applies.

Waiting periods:

No benefit is payable unless the policy has been in force for six months. The waiting period does not apply to death resulting from accidental causes. A waiting period of six months applies to new Secondary Assureds or revised benefits. Death resulting from suicide within two years of the policy does not qualify for a benefit.

Free-Look Period:

The Primary Assured can cancel the policy within 15 days of its inception by notifying Naa Sika in writing.

Currency:

Benefits under this policy are payable in Ghana Cedis.

Law:

The laws of the Republic of Ghana govern this policy.

Complaint Procedure:

Complaints should be made in writing to the company. If the matter is not resolved satisfactorily, the complainant can refer the complaint to the National Insurance Commission.

Exclusions:

No benefit will be payable for the death of the Primary Assured or any Secondary Assured(s) resulting from the following causes:

-War, invasion, acts of foreign enemies, terrorism, civil war, rebellion, or any acts directed towards overthrowing a government by force or through terrorism.
-Attempted suicide or self-inflicted injury, regardless of mental state.
-Any illegal activities or assaults provoked by the Assured.
-Consumption of alcohol or drugs, except as prescribed by a registered medical practitioner.
-Mental illness or disease.
-Military service or training in any country's armed forces, including army, navy, and air force.


Claims Procedure:

The Nominated Beneficiary or Primary Assured must inform Naa Sika of the death within a reasonable period.
Required documents include a burial permit, death certificate, medical report, policy document, police report if death results from an accident and identification cards.

SIC Life will pay the appropriate claimant within two working days after receiving all requested documentation.

Definitions:

Accidental death: Death caused solely and directly by violent, external, accidental, and visible means.
